SMAS Lifting · quick answer
What SMAS lifting means, in one paragraph
The SMAS (superficial musculo-aponeurotic system) is a fibrous layer under the skin and fat that supports the face; when it loosens, cheeks and jowls descend. A surgical SMAS facelift tightens this layer through incisions and is performed by plastic surgeons. Non-surgical SMAS lifting uses focused ultrasound (HIFU), such as Ultherapy, to heat points in the SMAS so it contracts and builds new collagen. Ever Skin Clinic, a dermatology clinic, offers the non-surgical approach and will tell you honestly if surgery would suit you better.

The treatment
What is SMAS lifting?
In a surgical SMAS facelift, a surgeon lifts and tightens the SMAS through incisions around the ears, which can correct significant sagging but involves anaesthesia and weeks of recovery. Focused ultrasound reaches the same layer from the skin surface without incisions: tiny thermal points cause the tissue to contract and remodel, producing a more modest, gradual lift.
At our clinic, SMAS lifting is usually performed with Ultherapy or other HIFU devices, often layered with radiofrequency for the skin surface.
Surgical vs non-surgical SMAS lifting
- Method: incisions and tissue repositioning versus focused ultrasound through the skin
- Degree of lift: surgery corrects heavier sagging; HIFU gives a subtler lift
- Recovery: weeks after surgery versus usually the same day with HIFU
- Duration: surgical results last years; HIFU is typically repeated about yearly
- Who performs it: plastic surgeons for surgery; our dermatologists for HIFU

Who is a good candidate?
Non-surgical SMAS lifting suits adults, often in their 30s to 50s, with early to moderate sagging who want a natural improvement without surgery. Patients with heavy jowls and very loose neck skin are more likely to benefit from a surgical facelift, and we will say so. Those who want added support may combine HIFU with thread lifting.
Non-surgical SMAS lifting may suit you if you
- See early jowls or a softening jawline
- Notice your cheeks sitting lower than before
- Want to delay or avoid a surgical facelift
- Need to return to work or travel quickly
- Want to maintain results after earlier lifting
Your visit
What happens during treatment
A full-face and neck HIFU session usually takes about 60 to 90 minutes, including preparation.
- Consultation A board-certified dermatologist assesses sagging, skin thickness and fat and explains whether non-surgical lifting is realistic for you.
- Mapping Treatment lines and depths are marked, avoiding nerves and areas that should keep their volume.
- Focused ultrasound Energy is delivered at the SMAS depth and often at shallower depths for skin tightening.
- Aftercare We explain what to expect as collagen builds and when to consider maintenance.
Real patient · before & after
Non-surgical SMAS lifting in practice
This patient had old filler dissolved with hyaluronidase and then received Shurink, a focused-ultrasound device that targets the SMAS layer. The result reflects both steps, not HIFU alone, and it is not a surgical facelift.
Your dermatologist will explain what non-surgical lifting can realistically achieve for your face.

Recovery, side effects and aftercare
Mild redness, swelling and tenderness, especially along the jawline, are common and usually settle within a few days. Less commonly, bruising, temporary numbness, nerve irritation, small welts or excessive fat loss can occur, so energy and depth are tailored to your face.
Tell your dermatologist if you have metal or electronic implants, open wounds or active acne in the area, recent fillers or threads, or are pregnant. Most travellers return to sightseeing the same day.
Aftercare tips
- Use a gentle moisturiser and daily sunscreen
- Avoid saunas and very hot baths for 1 to 2 days
- Take simple pain relief if advised for tenderness
- Avoid facial massage for about a week
- Plan maintenance about every 6 to 12 months
Questions we hear every day
SMAS Lifting FAQ
Do you perform surgical SMAS facelifts?
No. Ever Skin Clinic is a dermatology clinic and offers non-surgical SMAS lifting with focused ultrasound and related treatments. If your sagging would be better corrected by surgery, your dermatologist will tell you so honestly.
Is non-surgical SMAS lifting painful?
Focused ultrasound can feel like deep heat or brief aches, particularly over bone. Numbing cream and energy adjustments help, and most patients tolerate it well. Tenderness along the jaw may last a few days.
How long do HIFU SMAS lifting results last?
Results build over about two to three months and often last around a year, varying with age, skin and lifestyle. Many patients have maintenance sessions every six to twelve months.
Can HIFU replace a facelift?
Not for everyone. HIFU gives a subtler lift that suits early to moderate sagging. Heavy jowls and loose neck skin usually respond better to surgery. Many patients use HIFU to delay or maintain results after surgery.
Can I have it during a short visit to Seoul?
Yes. There is usually little downtime, so most patients return to normal activities the same day, and results continue to develop after you go home. Avoid scheduling it immediately before important photos in case of mild swelling.
