What are nasolabial fold fillers?
Everyone has nasolabial folds; they become more noticeable as cheek fat descends, bone support at the base of the nose recedes and skin loses elasticity. Filling only the line can make the area look puffy when smiling. A layered approach places supportive filler at the base of the fold near the nose and, where needed, in the cheek, then refines the crease itself with a softer gel.
For many patients, midface cheek filler does more for smile lines than direct filling. Where sagging is the main cause, lifting such as thread lifting or Ultherapy may be combined.

Recovery, side effects and aftercare
Mild swelling, tenderness and bruising are common and usually settle within a few days. Lumps or asymmetry can occur and are usually correctable. The area beside the nose contains an important artery, so vascular complications, though rare, are a known risk and are reduced by careful depth, cannula technique and conservative amounts.
Tell your dermatologist if you are pregnant or breastfeeding, take blood thinners, have cold sores, an active infection, an autoimmune condition or previous filler in the area. Contact the clinic immediately for increasing pain, colour change or vision change.
Aftercare tips
- Avoid pressing or massaging the area unless advised
- Avoid exaggerated facial expressions and dental treatment for a few days
- Skip saunas, hot baths and intense exercise for 2 to 3 days

Why not just fill the line itself?
Filling only the crease can look puffy or heavy, especially when smiling, because the fold is mainly caused by midface volume loss. Restoring support at the base of the fold and cheek usually looks more natural.
Can heavy smile line filler from another clinic be fixed?
Often yes. HA filler can be dissolved with hyaluronidase, and after it settles, your dermatologist can plan a more balanced re-treatment, as in the case shown on this page.
