Calf Botox · quick answer
What calf Botox does, in one paragraph
Calf Botox is an injection of botulinum toxin into the gastrocnemius, the large muscle that forms the bulge at the back of the calf. As the muscle is used less, it gradually becomes thinner, so muscular calves can look slimmer and straighter over a few months. It only reduces muscle, not fat or swelling. At Ever Skin Clinic, a board-certified dermatologist checks whether your calf shape comes from muscle, fat or bone before planning the dose and injection pattern.

The treatment
What is calf Botox?
The calf has two main muscles: the gastrocnemius near the surface, which gives the calf its rounded bulge, and the soleus underneath, which does much of the work in standing and walking. Calf Botox targets the bulky part of the gastrocnemius so that its volume reduces while the soleus keeps supporting everyday movement.
Calf Botox is one part of body Botox. If your calves look full because of fat or fluid rather than muscle, our dermatologists may suggest body slimming injections or other contouring options instead.
What calf Botox can and cannot do
- Can slim calves that are wide because of muscle bulk
- Can soften an uneven or outward-curving calf line
- Cannot remove fat or reduce fluid swelling
- Cannot change bone shape or leg length
- Works gradually , so it is not a quick fix for an event next week
Is it right for you?
Who is a good candidate?
The best candidates have firm, muscular calves that bulge clearly when they rise onto their toes. During consultation your dermatologist will examine your legs standing and on tiptoe, feel the muscle and discuss your activity level, because athletes, runners and people who rely on calf power need careful dosing.
Calf Botox may suit you if you
- Have calves that look bulky or rounded from muscle
- Notice one calf is larger than the other
- Want a straighter, slimmer lower leg line without surgery
- Do not depend on maximum calf power for sport or work
- Are willing to wait several weeks for the result
Your visit
What happens during treatment
The injections are quick, so calf Botox fits easily into a short stay in Seoul.
- Assessment Your dermatologist examines the calf at rest and on tiptoe to map the bulkiest areas of each muscle.
- Marking Injection points are marked over the inner and outer heads of the gastrocnemius on both legs.
- Injection The planned dose is spread across several points per leg using a fine needle.
- Follow-up We explain what to expect and can review your shape after about 4 to 8 weeks.

The same principle, a different muscle
This image shows masseter Botox, where a bulky jaw muscle was relaxed to slim the lower face. It is not a calf result, but it shows the same principle: Botox reduces the size of an overactive muscle gradually over several weeks.
Ask us at consultation for calf-specific examples and for a realistic estimate of how much your calves are likely to change.

Recovery, side effects and aftercare
You can walk normally straight after treatment. Mild soreness, small bruises or a feeling of tired calves on stairs, slopes or long walks are common in the first weeks and usually ease as you adapt. Excessive doses can affect running or jumping, which is why dosing is individual.
Tell your dermatologist if you are pregnant or breastfeeding, have a neuromuscular condition, take blood thinners or aminoglycoside antibiotics, have a history of leg clots, or have major sport or hiking plans in the coming weeks.
Aftercare tips
- Avoid massage or tight compression on the calves for the rest of the day
- Skip intense leg workouts and saunas for 1 to 2 days
- Choose comfortable flat shoes if your calves feel tired
- Avoid starting a heavy calf-training program while the muscle slims
- Plan maintenance sessions every 4 to 6 months if you want to keep the shape
Questions we hear every day
Calf Botox FAQ
Will calf Botox affect my walking?
Walking is mainly supported by the soleus muscle, which is largely spared, so most patients walk normally. Your calves may feel tired on stairs, slopes or long walks for a few weeks. If you run, hike or play sport seriously, tell your dermatologist so the dose can be adjusted.
When will my calves look slimmer?
The muscle needs time to reduce in size. Many patients notice a change from about four to eight weeks, with a fuller effect at around two to three months. Taking photos before treatment helps you compare, because gradual change can be hard to notice day to day.
How long do calf Botox results last?
Results typically last about four to six months, depending on the dose and how actively you use your calves. With repeated sessions, some patients find the muscle stays slimmer for longer between treatments, but the effect is not permanent and gradually fades without maintenance.
Is calf Botox painful?
Most people feel brief pinches and a mild ache as the medicine enters the muscle. The session is short, and numbing cream or ice can be used if you are sensitive. Mild soreness afterwards is common and usually passes within a day or two.
Can I have calf Botox if I am only in Seoul for a few days?
Yes. The treatment takes one short visit and has little downtime. Because slimming develops over several weeks, you will see the result after you return home. Avoid long hikes or intense leg exercise for the first couple of days, and take photos before treatment to compare.
